PERSONAL INFORMATION COLLECTED
The term “personal information” means any information about you that is personally identifiable, such as your name, address, email, telephone number, and other contact information. “Personal information” also includes any medical or lifestyle data that you provide to EBMapps if that information is linked to personally identifiable information, that is, if the medical or lifestyle data is associated with an identifiable individual. EBMapps collects various types of personal, demographic and non-personal information from users to help provide a superior user experience and to administer our Services.
For example, to become a member and use the paid features of the Services we may need to collect your name, home and business address or other contact address, email address, Social Security Number, medical benefits information, contact phone number, email address, and employment information, such as hire date, termination date, division, location, salary, etc.
Some of this information may be required in order to obtain medical benefits for the use of the Services. We will not pass any of your personal information to a medical benefits provider without your consent. If you authorize us to provide any personal information to a medical benefits provider, such as health insurance reimbursement for the use of the Services, we will only provide the minimum information necessary for the requested benefit.
We also may require you to create a unique password and/or security PIN and provide answers to security questions which we will retain on file for identification purposes.
The use of the Services generally requires us to collect medical and health-related information that you can provide, such as your body weight, blood pressure, blood sugar levels, and results of other blood tests from your doctor. You will also be asked about your consumption of food, beverages, smoking, use of alcoholic beverages, exercise habits and actual exercise performed, and other lifestyle indicia. This information will be kept strictly confidential. However, you may be given the option, with your permission, to give this information to a health care provider. Furthermore, we may anonymize any of your information and use information you provide for statistical purposes and aggregated medical studies in a manner that does not link any personally identifiable data to any of the medical or lifestyle information discussed in this paragraph.
In addition, we may collect or receive certain health-related information about you from time to time. In some instances, you may provide that information directly, such as when you fill out a form, send us a message, or connect a monitoring device which submits information about you electronically.
EBMapps may periodically ask you to complete short surveys about your experiences (including questions about products and services) for research purposes and/or on behalf of third parties working with EBMapps. You are free to respond or not. Your participation in these surveys is not required, and your refusal to do so will not impact your experience on the site.
In addition, if you choose to send EBMapps email inquiries or feedback or contact Customer Support, EBMapps will collect your personal information including your email address.
AUTOMATED COLLECTION OF NON-PERSONAL INFORMATION
COOKIES
Cookies are small data files associated with web browsers and mobile applications that enable our systems to recognize your browser and tell us how and when pages on our websites and mobile apps are used. Our use of cookies may be necessary for the Services to function as intended. For example, cookies may make the Services easier for you to use, may allow the Services to remember whether or not you are currently logged in, and may allow us to determine recent pages you visited, which enhances your user experience. Cookies in the EBMapps services do not collect personal information, and we do not combine information collected through cookies with other personal information to tell us who you are or what your email address is.
The “Help” option of the toolbar on the majority of Internet browsers will direct you on how to prevent your browser from accepting new cookies, how to command the browser to tell you when you receive a new cookie, or how to fully disable cookies. We do not recommend disabling cookies as certain parts of the Services will not function properly without cookies.

HOW WE SHARE PERSONAL INFORMATION
Our policy is to keep your personal information confidential unless you consent to releasing it to a third party. Please be assured that your personal information will be used only for the expressed purpose of the programs or services that you elect to participate in. We will not share your personal information with any party (either internally or externally) who does not have a need for such information in order to directly provide the programs or services that you choose to utilize.
We may ask if you consent to disclosing your personal information for the purposes of communicating eligibility and enrollment information to your employer and/or the applicable insurance broker or carrier, vendor, third-party plan administrator or other third party to obtain benefits to pay for all or a portion of the Services.
We may disclose your information to a third party in connection with a corporate reorganization, merger, sale, joint venture, assignment, transfer or other disposition of all or any portion of our business, assets or stock, including in connection with any bankruptcy or similar proceedings.
Except as provided above in this section, we will not share your personally identifiable information or program activity with anyone without your prior consent.
CONFIDENTIALITY AND SECURITY
Your EBMapps Services account information is protected by a password for your privacy and security. EBMapps requires users to adhere to good security practices in the selection and use of passwords, including: minimum length, complexity rules, history rules, and lockout.
EBMapps has adopted commercially reasonable technical precautions taken to help keep your data secure. Included in this is protecting member health information with 3DES encryption, and securing web traffic using 256-bit SSL encryption.
Despite these efforts, please understand that no system is perfect or can guarantee that unauthorized access or theft of data might not occur.
